Nursing Assessment

Nursing assessment is the gathering of information about a patient's physiological, psychological, sociological, and spiritual status by a licensed Registered Nurse.  Nursing assessment is the first step in the Nursing process.

Home Health Aide Services

"Home Health” refers to medical services provided at the patient’s residence.  The residence can be a private home or assisted living facility.  The home health services typically include: skilled nursing, physical therapy, occupational therapy, medical social work and in-home aide.  Home health may also include medical equipment and supplies.

Matching Care

Finding the right fit between patient and aide is second only in importance to providing the finest care.  The process is gradual and if, at any point, the client or family feels a change should be made, our Care Coordinators will do so in a way that is respectful to all parties.  The key ingredients of good home care are medical excellence and personal kindness. 

Aides

All aides are responsible to provide training certificates, licenses or degrees, and identification before the application process.  Each applicant must pass a written examination and skills evaluation, and clear two interviews.  All references, including criminal background checks, fingerprints are verified by the Department of Health and Government Agencies.  All staff members must attend a 5 hour orientation and sign a code of conduct policy.

Communication

Good communication is essential.  One of the most important  aspects is the necessity of open, complete, timely and accurate communication among and between those who provide care to a patient in the home.  We pride ourselves on our commitment to answering all calls personally (No Answering Machines/Outsourced Call Services).  All team members are trained to direct you to the immediate support you need to answer any questions or concerns.

Cost and Payments

The plan of care for every client is unique and is customized accordingly.  There are several factors that go into determining the cost: the level of service required (Companion, HHA, transportation, requested services, etc) and the number of hours of care you receive (Hourly/Live-in).  Our service costs are suitable to your budget and we will work with you on a financial plan to insure you receive the care you need. Invoicing is bi-weekly and will be sent out directly for payment of services.  We work with long-term care insurance and workman's compensation.
